- Identification of Material Removed: A description of the material that was removed and its location before removal.
- Statement of Good Faith: A statement under penalty of perjury that you have a good-faith belief the material was removed or disabled as a result of mistake or misidentification.
- Consent to Jurisdiction: A statement that you consent to the jurisdiction of the federal court in your area or, if outside the United States, the court closest to our office location, and agree to accept service of process from the person who filed the initial DMCA notification or an agent of that person.
